Missing/Invalid Birthdate

Rejection Message:

  • ACKNOWLEDGEMENT/REJECTED FOR INVALID INFORMATION^Entity's date of birth^Patient

This rejection states that the date of birth on the claim does not match what insurance has on file for the patient. To fix this rejection it would be beneficial to follow up with the patient and/or insurance to make sure that all patient information is accurate. Once the patient information has been updated, the claim can be rebuilt and resubmitted.
