Clinical Quality Measures Report - Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents

Overview
Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physicians (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported. 1- Percentage of patients with height, weight, and body mass index 2- Percentage of patients with counseling for nutrition 3- Percentage of patients with counseling for physical activity

A patient will be in the denominator when they have all of the following- 

  • An age from 3-17 years old
  • A visit during your reporting period

A patient will be in the first numerator when they have all of the following- 

  • A BMI Percentile LNC code entered or imported on a Lab order with a Received Date within your reporting period
  • Height LNC code entered or imported on a Lab order with a Received Date within your reporting period
  • Weight LNC code entered or imported on a Lab order with a Received Date within your reporting period

A patient will be in the second numerator when they have all of the following- 

  • Counseling for Nutrition procedure on their ledger on the same day as their visit, or snomed code added to their EMR or EHR from with a Start Date and End Date matching the Procedure Date of their visit

A patient will be in the third numerator when they have all of the following- 

  • A Counseling for Physical Activity snomed code in the EMR or EHR form with a Start Date and End Date matching the Procedure Date of their visit

Stratifications

  • Stratum 1 measures are calculated in the same manner for patients 3-10 years old
  • Stratum 2 measure are in the same manner for patients 11-17 years old
 A patient will be in the exclusions when they have all of the following- 
  • A Pregnancy diagnosis with an Onset Date within your reporting period
