Clinical Quality Measures Report - Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention

Overview

Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months AND who received tobacco cessation intervention if identified as a tobacco user

Three rates are reported:
a. Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months
b. Percentage of patients aged 18 years and older who were screened for tobacco use and identified as a tobacco user who received tobacco cessation intervention
c. Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months AND who received tobacco cessation intervention if identified as a tobacco user

A patient will be in the first denominator when they have all of the following- 

  • An age of 18+ years old
  • A visit during your reporting period if seen for- Preventive Care Services- Group Counseling, Preventive Care Services- Other, Preventive Care Services- Initial Office Visit, 18 and Up, Preventive Care Services- Established Office Visit, 18 and Up, Preventive Care Services- Individual Counseling, Face-to-Face Interaction, Annual Wellness Visit, or Speech and Hearing Evaluation

OR

  • An age of 18+ years old
  • Two visits during your reporting period if seen for- Psych Visit - Diagnostic Evaluation, Health and Behavioral Assessment - Initial, Health & Behavioral Assessment - Reassessment, Health & Behavioral Assessment - Individual, Occupational Therapy Evaluation, Office Visit, Psych Visit - Psychotherapy, Psychoanalysis or Ophthalmological Services, Ophthalmological Services, Home Healthcare Services

A patient will be in the first numerator when they have all of the following- 

  • A Tobacco Use Screening LNC code entered as a Laboratory Order test and a Received Date within the last 24 months prior to the End Date of your reporting period 
  • A Tobacco User or Tobacco Non-User snomed code entered in as a Result code on that LNC code

A patient will be in the second denominator when they have all of the following- 

  • All of the above with an indication of tobacco use (via the snomed code)

A patient will be in the second numerator when they have one of the following- 

  • A Tobacco Use Cessation Counseling snomed code entered in the EMR or EHR form with a Start Date and End Date matching the dates of the Tobacco User snomed code
  • A Tobacco Use Cessation Pharmacotherapy medication with an Ordered Date within the last 24 months prior to the End Date of your reporting period

A patient will be in the third denominator when they have all of the following- 

  • An age of 18+ years old
  • A visit during your reporting period if seen for- Preventive Care Services- Group Counseling, Preventive Care Services- Other, Preventive Care Services- Initial Office Visit, 18 and Up, Preventive Care Services- Established Office Visit, 18 and Up, Preventive Care Services- Individual Counseling, Face-to-Face Interaction, Annual Wellness Visit, or Speech and Hearing Evaluation

OR

  • An age of 18+ years old
  • Two visits during your reporting period if seen for- Psych Visit - Diagnostic Evaluation, Health and Behavioral Assessment - Initial, Health & Behavioral Assessment - Reassessment, Health & Behavioral Assessment - Individual, Occupational Therapy Evaluation, Office Visit, Psych Visit - Psychotherapy, Psychoanalysis or Ophthalmological Services, Ophthalmological Services, Home Healthcare Services

A patient will be in the third numerator when they have one of the following- 

  • A Tobacco Use Screening LNC code entered as a Laboratory Order with a Value and a Received Date within the last 24 months prior to the End Date of your reporting period 
  • A Tobacco User or Tobacco Non-User snomed code entered in as a Result code on that LNC code
    • A Tobacco Use Cessation Counseling snomed code entered in the EMR or EHR form with a Start Date and End Date matching the dates of the Tobacco User snomed code
    • A Tobacco Use Cessation Pharmacotherapy medication with an Ordered Date within the last 24 months prior to the End Date of your reporting period

A patient will be in the exceptions when they have all of the following- 

  • A diagnosis of Limited Life Expectancy with an Onset Date before the End Date of your reporting period
