Overview
Percentage of patients aged 20 through 79 years whose risk factors have been assessed and a fasting LDL-C test has been performed.
High Risk
All patients aged 20 through 79 years who have CHD or CHD Risk Equivalent OR 10-Year Framingham Risk > 20%
- Denominator 1:
- The patient needs to be 20-79 years old
- They need to be seen during the reporting period, indicated with the listed Cholesterol Measure Encounter CPT codes in the report footer
- They need an active CHD or CHD Risk Equivalent diagnosis, indicated with the listed ICD-9, ICD-10, or snomed codes in the report footer
- Numerator 1:
- The patient needs one of the following entered or imported as a Laboratory Order, with a Received Date within the filtered date range-
- LDL Code
- Total Cholesterol, High Density Lipoprotein (HDL), and Triglycerides
- The patient needs one of the following entered or imported as a Laboratory Order, with a Received Date within the filtered date range-
- Exclusions:
- Any patient who is diagnosed as pregnant, indicated with the listed ICD-9,ICD-10, or snomed codes in the report footer, is excluded from the report.
Moderate Risk
All patients aged 20 through 79 years who have 2 or more Major CHD Risk Factors OR 10-Year Framingham Risk 10-20%
- Denominator 2:
- Numerator 2:
- The patient needs one of the following entered or imported as a Laboratory Order, with a Received Date within the filtered date range-
- LDL Code
- Total Cholesterol, High Density Lipoprotein (HDL), and Triglycerides
- The patient needs one of the following entered or imported as a Laboratory Order, with a Received Date within the filtered date range-
- Exclusions:
- Any patient who is diagnosed as pregnant, indicated with the listed ICD-9, ICD-10, or snomed codes in the report footer, is excluded from the report.
Low Risk
All patients aged 20 through 79 years who have 0 or 1 Major CHD Risk Factors OR 10-Year Framingham Risk <10% ** For Denominator 2 and Denominator 3, Fasting HDL-C > or equal to 60 mg/dL subtracts 1 risk from the above (This is a negative risk factor.)
