Insurance Report - Patients By Insurance and Appointment Type

The Patients By Insurance and Appointment Types Report lists patients with appointment types on appointments, sorted by the Insurance Company associated to the patient.

This documentation contains the following sections:

Filters

  • Providers: Filters the results based on the provider listed in the Patient Ability > Patient Tab.
  • Offices: Filters the report based on the office listed on the Patient Ability > Patient Tab.
  • Insurances: Filters the report based on specific Insurances as listed in the Insurance Companies Reference.
  • Appointment Type: Filters the report based on Appointment Type.
  • Appointment Start/End Date: Filters the report by appointment date based on a set date range.
  • Appointment Date/Ignore Date Range:
  • Include Inactive Patients: This checkbox can be checked to include Inactive patients.

Results

  • Carrier Name: The name of the Insurance Company.
  • Address: The Address of the Insurance Company.
  • Phone: The Phone number of the Insurance Company.
  • # of Patients: The number of patients which have Appointment Types for this Insurance Company result.

  • Patient #: The patient number for the patient with the Appointment Type within the Insurance Company result.
  • Patient Last, First (Age, Sex): The first and last name, age, and sex of the patient with the Appointment Type within the Insurance Company result.
  • Subscriber Num: The Subscriber Number of the patient with the Appointment Type within the Insurance Company result.
  • Group #: The Group number for the patient associated to the Insurance Company result.
  • Address: The address for the patient.
  • Phone: The phone number for the patient.
  • Plan Name: The Plan Name for the Insurance Company result.
  • Office/Provider: The Office/Provider associated to the Patient.
