Overview
Percentage of patients aged 12 years and older screened for clinical depression on the date of the encounter using an age appropriate standardized depression screening tool AND if positive, a follow-up plan is documented on the date of the positive screen.
A patient will be in the denominator when they have all of the following-
- An age of 12+ years old
- A visit during your reporting period
A patient will be in the numerator when they have all of the following-
- An Adolescent Depression Screening (12-18) or Adult Depression Screening (18+) LNC code entered as a Laboratory Order with a Received Date matching the Procedure Date of their visit
- Either a Negative Depression Screening or Positive Depression Screening snomed code entered as a result (using the Blank option when searching for a code) on the Depression Screening LNC code
- If they have a Positive Depression Screening, they also need one of the following
- A Depression Medication with an Ordered Date matching the Procedure Date of their visit
- An Additional Evaluation for Depression snomed code entered in the EMR or EHR form with a Start Date and End Date matching the Procedure Date of their visit
- A Referral for Depression snomed code entered in the EMR or EHR form with a Start Date and End Date matching the Procedure Date of their visit
- A Followup for Depression snomed code entered in the EMR or EHR form with a Start Date and End Date matching the Procedure Date of their visit
- A Suicide Risk Assessment snomed code entered in the EMR or EHR form with a Start Date and End Date matching the Procedure Date of their visit
- If they have a Positive Depression Screening, they also need one of the following
