Clinical Quality Measures Report - Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan

Overview
Percentage of patients aged 18 years and older with an encounter during the reporting period with a documented calculated BMI during the encounter or during the previous six months AND when the BMI is outside of normal parameters, follow-up plan is documented during the encounter or during the previous 6 months of the encounter with the BMI outside of normal parameters.

A patient will be in the denominator when they have all of the following- 

  • An age of 18+ years old
  • A visit during your reporting period

A patient will be in the numerator when they have all of the following- 

  • A BMI Percentile LNC code entered or imported on a Lab order with a Value and a Received Date within your reporting period
    • If the BMI is above or below normal, the patient needs one of the following-
      • An Above/Below Normal Follow-up visit within 6 months of their original visit
      • An Above/Below Normal Medication with an Ordered Date within 6 months of their original visit
      • A Referrals where weight assessment may occur snomed entered in their EMR or EHR form with a Start Date and End Date within 6 months of their original visit

A patient will be in the exclusions when they have all of the following- 

  • A Pregnancy diagnosis with an Onset Date within your reporting period
