Clinical Quality Measures Report - Documentation of Current Medications in the Medical Record

Percentage of specified visits for patients aged 18 years and older for which the eligible professional attests to documenting a list of current medications using all immediate resources available on the date of the encounter. This list must include ALL known prescriptions, over-the-counters, herbals, and vitamin/mineral/dietary (nutritional) supplements AND must contain the medications' name, dosage, frequency and route of administration.

A patient will be in the denominator when they have all of the following- 

  • An age 18 years or older
  • A visit during your reporting period (every unique visit will be listed, so patients can show up in the denominator multiple times)

A patient will be in the numerator when they have the following- 

  • A Current Medications Documented snomed code added to their EMR or EHR from in the snomed code section with a Start Date and End Date matching the Procedure Date of their visit
