Clinical Quality Measures Report - Diabetic Retinopathy: Communication with the Physician Managing Ongoing Diabetes Care

Overview
Percentage of patients aged 18 years and older with a diagnosis of diabetic retinopathy who had a dilated macular or fundus exam performed with documented communication to the physician who manages the ongoing care of the patient with diabetes mellitus regarding the findings of the macular or fundus exam at least once within 12 months

A patient will be in the denominator when they have all of the following- 

  • An age of 18+ years old
  • A visit during your reporting period 
  • A Diabetic Retinopathy diagnosis with an Onset Date matching the visit's Procedure Date
  • A Macular Exam added to a Laboratory Order with a Value and a Received Date matching the visit's Procedure Date

A patient will be in the numerator when they have all of the following- 

  • A Medical Records Transfer snomed added to their EMR or EHR from in the snomed code section with a Start Date and End Date after the visit's Procedure Date
  • A Macular Edema Findings Present or Macular Edema Findings Absent snomed added to their EMR or EHR from in the snomed code section with a Start Date and End Date after the visit's Procedure Date
  • A Level of severity of Retinopathy Findings snomed added to their EMR or EHR from in the snomed code section with a Start Date and End Date after the visit's Procedure Date
