*RETIRED* Clinical Quality Measures Report - Colon Cancer: Chemotherapy for AJCC Stage III Colon Cancer Patients

Overview
Percentage of patients aged 18 through 80 years with AJCC Stage III colon cancer who are referred for adjuvant chemotherapy, prescribed adjuvant chemotherapy, or have previously received adjuvant chemotherapy within the 12-month reporting period.

  • Denominator:
    • The patient must be 18-80 years old
    • They must be diagnosed with Colon Cancer, indicated with the listed ICD-9, ICD-10, or snomed codes in the report footer
    • They must be seen at least twice during the filtered date range and after the Onset Date of their Colon Cancer diagnosis, indicated with the listed CPT codes in the report footer
    • They must have a a Clinical Staging Procedure snomed code entered on their EMR or EHR form, with a Start Date somewhere between the Onset Date of their Colon Cancer diagnosis and the Procedure Date of their first Office Visit CPT code.
    • This Clinical Staging Procedure needs a Colon Distant Metastasis Status M0 snomed codes entered as the CQM Value
    • They need two additional Clinical Staging Procedure snomed codes added in the same manner with the same Start Date. One needs to be Colon Cancer Primary Tumor Size T1, T2, T3, T4a, or T4b, and the other needs to be Colon Cancer Regional Lymph Node Status N1, N1a, N1b, N1c, N2a, or N2b. They can also be the combination of Colon Cancer Primary Tumor Size T4b and Colon Cancer Regional Lymph Node Status N2.
  • Numerator:
    • The patient needs one of the following combinations of medications prescribed after their Clinical Staging Procedure and before their Office Visit CPT
      • Chemotherapy for Colon Cancer - capecitabine
      • Chemotherapy for Colon Cancer - fluorouracil and Chemotherapy for Colon Cancer - leucovorin and levo-leucovorin
      • Chemotherapy for Colon Cancer - capecitabine and Chemotherapy for Colon Cancer - oxaliplatin
      • Chemotherapy for Colon Cancer - fluorouracil, Chemotherapy for Colon Cancer - leucovorin and levo-leucovorin, and Chemotherapy for Colon Cancer - oxaliplatin
