Additional Report - NYC Immunization Data

**Note- this report is for historical reference only. NYC no longer accepts immunizations data in this manner and hasn't for quite some time. If you'd like to send immunizations data to NYC (or to your own state registry), please contact MacPractice support to get started with the interface process.**

 

The NYC Immunization Data report displays immunization data specifically for the electronic transmission of vaccinations to the New York State Immunizations Information System (NYSIIS).

The report collects Immunization data as specified by the NYSIIS's Flat File Transfer Specification.

The report can be filtered by Providers, Offices, or Vaccination Start and End Date.

For an Immunization to appear within the report results, the Immunization must be added to the Clinical tab in the patient's Immunizations table. The immunization within the table must also have a specified provider and office. The status must be set to Complete. Per the specifications of NYSIIS, the electronic transmission of the immunization records must also have a Record Identifier (CVX code), date, and CPT or Vaccine Group code.

Additional information that is not currently required but specified by the NYSIIS's Flat File Transfer Specification is also listed within the report. Where information is not available for a specified field, NYSIIS specifies that the entire field must be blank. Where this information is not used within MacPractice, the field has been left blank. Contact the NYSIIS Help Desk for data reporting on these fields.

The following columns are included in the report:

  • Record Identifier: This required information is the CVX code as set in the immunization record within theImmunization node in the References ability.
  • Vaccine Group: The electronic transmissions must include either a Vaccine Group code or a valid CPT code. MacPractice uses the CPT code. Vaccine Group Codes are not used by MacPractice. Contact the NYSIIS Help Desk for data reporting using Vaccine Group Codes instead of CPT codes.
  • CPT Code: This required information is set in the immunization record within the Immunization node in the References ability.
  • Trade Name: This information is related to the NYSIIS code set for Vaccine Codes. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
  • Vaccination Date: The date in this column is automatically populated based on the date the immunization record was created in the Patient's Clinical tab within the Immunizations table.
  • Administration Route Code: This information is related to the NYSIIS code set for Administration Route. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
  • Body Site Code: This information is related to the NYSIIS code set for Body Site. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
  • Reaction Code: This information is related to the NYSIIS code set for Reaction. This information is not used by MacPractice. While a Reaction statement can be added to the Immunization table in the patient's Clinical tab, the two are not related.
  • Manufacturer Code: This information is set in the immunization record within the Immunization node in the References ability.
  • Immunization Information Source: This information is related to the NYSIIS code set for Immunization Information Source. This information is not used by MacPractice and will be blank within the report. The NYSIIS may default this information to historic values if not reported directly to the organization. If the immunization was performed by your office, contact the NYSIIS Help Desk for data reporting.
  • Lot Number: This information is set in the Immunizations table of the patient's Clinical tab, within the Lot Number column.
  • Provider Name: This information is set in the Immunizations table of the patient's Clinical tab, within the Provider column.
  • Administered By Name: NYSIIS considers this information to be the name of the person who administered the vaccination. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
  • Site Name: This information is set in the Immunizations table of the patient's Clinical tab, within the Office column.
  • Sending Organization: This is ID of the provider organization that owns this patient and corresponding immunization records. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting and the appropriate organization ID.
  • Financial Class: NYSIIS considers this information the appropriate HL7 table 0064 values for Financial Class. This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
  • Vaccine Purchased With: NYSIIS considers this information the appropriate HL7 values for funding type (private or public funds). This information is not used by MacPractice. Contact the NYSIIS Help Desk for data reporting.
To export the report, click the Export button. If an entry within the report appears in red, the related entry is invalid. The Export button will be disabled in the event that any invalid entries are found.
