Typically, this message occurs when a Prior Authorization is required on the eClaim in question. It could be that the Prior Authorization is out of visits, or the included Prior Authorization is past the expiration date set.

If a Prior Authorization was not added to the claim in question, you'll need to create a new claim and remember to add in the Prior Authorization as described in the linked article above.
