How do I add an old diagnosis code to a patient chart?

In MacPractice, we keep track of all the latest changes to ICD-10 diagnosis codes from year to year to ensure that your practice is always able to use the latest official ICD-10-CM and ICD-10-PCS codes. But what if you need to refer to an older code that was invalidated when the diagnosis codes were updated?

The answer is to add those codes in as a Favorite.

Using Favorites to add older ICD-9 or 10 codes

First, you will need to navigate to References > Favorites. You'll notice two different Favorites categories, one for ICD-9, one for ICD-10. We will be using ICD-10 as an example, so if you need to address ICD-9 diagnosis codes, these instructions will be mostly identical. you will just need to ensure your Preferences > Coding > Problems/Billing Diagnoses are set to ICD-9, and add your Favorites into the Favorites ICD-9 Diagnoses.

With the Favorites ICD-10 Diagnoses category selected, click on the green plus in the sidebar. This will bring up the search window for diagnosis codes.

This search dialogue covers both outdated codes and the new codes. For example, you can see here in the above screenshot that code D49.5 is present. This was a valid code in 2016 and was deleted in 2017. If we wanted to add this code in as a favorite, we would simply select the code in question, and click 'OK'.

After adding this code, you can then adjust the code's Alias, Diagnosis Type, Custom Description, add in comments and mark it inactive/non-billable. For more information on these options, please refer to References - Favorites ICD-10 Diagnoses or References - Favorites ICD-9 Diagnoses.

Once the desired code is added, you can then add this code into either the Problem List in the Patient > Clinical tab, or you can associate the diagnosis code to a ledger charge.

To add a diagnosis code to the Problem List, navigate to Patients Ability > Clinical > Problem List, and then click the green plus on the right side of the problem list. Once the search window appears, click on the 'Favorites' tab. All codes listed in the Favorites section will appear here, as long as your Coding Preference is set to ICD 10.

To add a Favorite diagnosis to a charge, navigate to Patients Ability > Ledger, and add a new charge. From there, click on the Diagnosis tab by the Diagnoses window, then click on the search button. Once the search window appears, click on the 'Favorites' tab. All codes listed in the Favorites section will appear here, as long as your Coding Preference is set to ICD 10.
